Declared permission scope
One scope per line (letters, digits, . : _ - *, up to 64 chars). When this list is non-empty, sealing a decision whose decision.permissions includes a scope outside the list automatically opens a permission_violation incident (the receipt is still sealed — evidence is never lost). Leave empty to disable the policy.
File a safety / governance incident
Record an event observed by your own monitoring: hallucinations, data exposure, unauthorized actions, human overrides. Do not paste raw user content into detail.
